Many of our local agencies have our PVSC application on file. Agencies providing the application form must fill out their agency name, mailing address and the name of a contact person in the Contributing Agency box on side 2 of the form. Agencies providing applicants with a request letter must do so on agency letterhead. The letter must contain the name and full mailing address of the agency, a contact person, the applicant's name, their role, who the vulnerable persons are that the applicant will be working or volunteering with and a statement that a PVSC is required. If the requesting agency has not provided the applicant with our application form or a request letter the applicant may attend one of our stations to fill out the form but the applicant must first contact the agency for the following information: confirm that a PVSC is the type of check required, find out the the name and full mailing address of the agency, the name of the contact person, applicant's role / title  and who the vulnerable persons are that they will be working or volunteering with. This information is required for the Niagara Regional Police Service (NRPS) to complete a PVSC. Applicants who obtain a letter will also be provided with a PVSC application form to complete at the police station when they present the letter.

Receipt of incomplete PVSC application forms by the NRPS Records & Information Management Unit will result in a delay until the information can be completed or verified by the applicant.

Once you have completed your PVSC application, obtained a letter or the required information from the requesting agency, please attend any of the following NRPS districts with the required documentation / information.
